Possible memory leak?

I am constantly getting the errors in the attached txt document.

Also it appears after a few days of the server running it starts to use
large amounts of memory and the servers ticrate drops through the toilet.
All the other servers I have one the box are still working fine the GG5
server is the only one effected. I think there might be a memory leak somewhere.
