Random Sound not firing on Multi Level (sometimes)

I am having a rather random error with using random sounds, no pun intended, for the multi level.

I have 22 custom files that the server randomly chooses every time some one gets a Multi Level. For the most part it works great every new Multi Level is a new random song. However, on occasion no sound is played. I have checked the logs and there is nothing, I have also had the players check their console and there are no errors there either. All songs have been individually tested and work. It seems that some times the person whom has the Multi Level can hear the sound and people close to them can as well, however people at a distance can not. Other times no one can hear the sound at all.

Anything I can do to help debug this, as always I will, just let me know what you need of me.

Done a lot more testing and it appears to be when you are killed by then some one kills the person whom killed you and triple levels, you don't hear the sound but everyone else does. Pretty weird. Any ideas?
